Chronic Care for Diabetes Patients

GR Diabetes & Metabolic Centre is dedicated to providing chronic care for patients with all types of diabetes. Our team offers doctor's consultations, diabetes education, dietician services and diabetes screening consultations. Your first visit with us will be an in depth 60-90min consultation. Full history taking, thorough physical examination, discussion on treatment options, lifestyle changes, and risk screening. Practical challenges will be addressed by our educator. You will need to see your doctor at least every 6 months, or more frequently if your diabetes is uncontrolled. On each visit Doctor will discuss your glucose control, screen for early signs of complications and do/refer you for regular screening tests.
